Thanks! I called them and the lady that handles revision is not so nice as if the didn't need my business. She said I will send you the information but DONT SEND IT BACK UNLESS IT IS COMPLETED TOTALLY I men yelling that's the best customer I have ever gotten. Lol😊😊😊

Theres no excuse for her rudeness, but if the documetation isn't complete, then it's worthless to them.  My friend went to Dr. Smith and loves him.  Office staff are often aloof at best and downright rude at times.  

Val is right, there is no excuse for rudeness. On the other hand, they really do need the paperwork properly filled out.

Dr. smith has an excellent reputation so I hope this encounter won't put you off.
