Met with the surgeon for the first time today...
Even though my BMI is slightly below 50, Dr. Alverdy had no qualms about about moving forward with the DS. I met with the dietician and psychiatrist as well. Alverdy used to be strict about the BMI rule, but has changed his mind after seeing great results in smaller patients over the years. With my comorbiditites he didn't see any reason NOT to choose the DS and thinks I'm a great candidate.
He didn't seem overly concerned about the insurance either; BCBS of IL wants a BMI >50 to cover the DS, but he expects they can appeal that and get around it. He said their insurance folks are great at getting it covered and they'll get it done. So I'm cautiously optimistic about that, too.
Overall, the appointment went as well as possible, and I'm feeling pretty good about everything. Just have to schedule a sleep study and wait on word from the insurance company...And maybe start weaning myself off the Diet Coke a bit, in preparation!

Wishing you all the best. BCBS is a beast, I hope Dr A is right.
