DS cost?

IF your insurance covers WLS, you will end up having to pay your entire deductible and your entire out of pocket maximum for the year (assuming none of it has been met already) plus any program fee that the surgeon charges.  In my case it was $2500 deductible + $4000 out of pocket maximum + $0 surgeon's program fee= $6500 total.  Of course everything else I had during my insurance plan year after that point was free (and I got a free endometrial ablation out of the deal).  It is best to have your surgery early in your insurance plan year for this reason.
