Please Help- I'm so frustrated!

Hi All,

I had a weight loss procedure called a bileopancreatic diversion with a duodenal switch aka BPDDS in May of 2010. Woo hoo I lost over 120 pounds. The surgery is malabsortive and you have significant increase in bowel movements. Well all those movements caused irritation and numerous hemaroids which were both prolapsed and thrombosis.

I went to a colorectal surgeon *****commended hemaroid surgery with the less invasive stapling technique. It was a brutal recovery which led to a 2nd surgery two weeks later and massive amounts of pain killers which caused a bowel obstruction and severe pancreatitis. I spent 23 days in the hospital.

On a side note I need neck and knee surgery which required me (to avoid surgery) to take additional long term pain meds and anti-inflammatories. Which caused rectal and stomach bleeding and another obstruction in February with a 2nd bout of pancreatitis in February.

At that time they found I was severely Anemic and I started iron infusion therapy. However my numbers have not risen or completely stabilized and vary between 9.7 and 11. My hematologist said that I can stop the treatments because I continue to bleed rectaly. I saw my GI guy and he did an upper endoscopy and a colonoscopy and said I had a few benign polyps and deep Hemaroids. When I saw the surgeon he said that he saw no Hemaroids with the colorectal camera.

My hematologist said the Hemaroids need to be fixed or I can't stop the infusions and I won't be able to have the neck and knee surgery. I bleed every day. I can't stand too long I can't sit because it hurts my rectum. I can't take pain killers because of the bleeding and the obstruction and pancreatitis. At this point I'm going crazy. I use anusal suppositories 2x a day follows by ointment and proctofoam. I have no idea where to turn. Any advice would be appreciated.

I had the same surgery, but I have not had any problems like you are mentioning. I wish I could help and maybe someone on here will be able to offer some advice.l

I hope you can find some relief from your situation.

Nov. 14/2005 I have reached the 10 year mark and I am very glad I had this surgery and can live a normal life.

I recommend a second opinion.  My mother had hemmoroid surgery and was still in horrible pain.  Her doc assured her that he got everything, and offered no other remedy.  She saw another doc, who advised that she had anal fissures that were causing the pain.  It required a second surgery - and it was a success!

I agree with Val - it's time for a second opinion. With or without a DS, no one can live with continued rectal bleeding and not become and remain anemic. And you should not have to live with this pain, either.

There are lots of general surgeons and colorectal surgeons with lots of experience with both hemorrhoids and fissures because both are common problems. get your ass (pun intended) to someone else and see what they say. At this point, the real problem isn't your DS, it's whatever was done and whatever still needs doing with your anal problems.

 second opinion time, for sure. However, I want to address your statement "The surgery is malabsortive and you have significant increase in bowel movements." How many BMs were you having? How many do you have now? A few months back we took a poll, and the 'average' DSer has 2 BMs pr day. If you're still having more than that---which would, no doubt, be a factor in your continuing anal problems---tell us what you're eating, and maybe we can help.

Thanks everyone for your responses. I did call and have a 2nd opinion scheduled for next Friday.

As far as Bm's I have was having a 6-8 small movements per day however i adjusted my diet to include mote protien and started having recral pain. After the rectal surgery, i stopped going completely. they said i had a bowel obstruction and i was hodpitalized for 23 days 21 of which i was clear liquids. I had to undergo a third surgery in the hospital to clear the blockage. i went back to a regular diet with reduced carbs and fats ate more protien and veggies less starch and fried foods. however as the rectal pain and my neck pain increased my use of painkillers increased. In October, I injured my knee at work. The result was cortisone shots, pain patches and anti-inflammatories.

Unfortunately, this created a 2nd blovkage and the 2nd bout of pancrestitis. i must say thst the hemroids were bothersome before...but now, my anus is too tight, I have thrombosis hemroids, I bleed almost every other day usually after pain in my bladder or left side, if I stand too long it feels like my ass is going to fall out but not at the anus, if I walk or sit to long it hurts. this is since tge rectsl surgeries.

In addition and not to be too graphic I'm afraid to have relations with my wife because I feel that I'll have an accident and its very painful to hold in gas let alone the embarrassment that it would cause. I brought all these issues up to the doc, saw a nutritionist, and have tried (although its very difficult) to curb my eating and make the right choices.

Any suggestions would help me greatly. I'm going through what seems to be hell right now. Declared unfit to teach due to my health, not fired but can't collect unemployment or govt help (I'm a sped teacher) wife was laid off. Times are tough now my daughter is suffering because she's witnessing this...all I need is a luck break and good advise. THANKS.
