Trouble? bones being leeched of calcium?
There isn't a standard but I use Vitalady's plan and tweak with my labs. I don't have to take any vitamin A or B-Complex, and take several of my vitamins just 3 days a week now. It's much easier to tweak down once you get your labs up, they tend to stay up once you get them up and supplement them a little less.
I'll have to get to the other computer and post a link for you. You can also go to www.vitalady.com and check out her pre-packaged plans.

Was your A really low to be taking 200k daily? Do you have the result of your D3?
You may want to take your dry vits either when you first wake up or during the night. You may be able to cut back to 1 Vit A-25k, and 1 Vit D-50k
Michelle had me add an extra calcium which brought me up to 6x/day to help bring my PTH down, but your D has to come up.
Why I started taking my dry vits away from food as I was taking 75k of A and 200k of D and both were trending down when I took them during the day. I started the middle of the night with 2-25k A of 3-50k D and my E. At 3 mo I tested and A & D were up. Went back to 1-25 A and 1-50k D and my levels have been great for the last 2.5 yrs.

My A right now is in normal range but low normal and is trending up consistently. My D was 125 then i lowered my dose to 70k and now it is 69. My D has never been a trouble to get up or down. The only thing that was slow to respond was A but since it is trending up I am not worried. I do not take my A, E, Zinc, K with food. They say to do that with normies but I figure its one more thing our intestines have to sort and absorb. I would rather not wake in the middle of the night to take pills. If I do not get good solid sleep, I am exhausted. Plus I am such a heavy sleeper that no alarm will wake me up without help. My poor husband would have to wake me up every time my midnight pills needed to be taken. I have my thyroid meds early in the morning and I can't take anything to soon by that. So I wait an hour after breakfast to take my first dose. I did plan it that I wouldn't be eating with any of my A, E, Zinc K.
I can only hope that when I get my A up that it will be easy to maintain like yours has been:) I did up my calcium intake by a bit over the past 3 months before my labs. Maybe more is needed. I would love to get the tests that Gina mentioned to get a better idea of what is going on.
Do you eat any fat before you go to bed? Maybe take just 2-A-25k just before you go to bed. I think taking my dry vits with food, that it just disappeared with my fat. Unfortunately, I'm up generally several times during the night, so it does make it easy to take 2 if not 3 calicum doses. I try to take my potassium, A, D, & E and once a week my K1 & K2. Kaiser won't test my K, but when I had my PS pre-op tests, the PTT was high. When I investigated, it was a low K, so I really up'd my K1 & K2 doses before surgery.

I am thinking I will try to swap around my vits in the evening so I do take my A right before bed. That might improve my numbers considerably, since i don't have any food after 7 usually. I will have to rehash this vit regimen and make it work. I might just have to bite the bullet and wake up in middle of night to take some A.
