form letter to find post ds op dr. explaining surgery

i had found sometime in dedember a letter to a potential pcp dr. explaiining the post op needs of a ds patient. it gave a diagrahm of the surgery and need for monotoring labs etc.

i cannot find it now, does anyone know where i can retain a copy? i am preparing to meet a potential new dr. next week!

waiting for catscan results...should find out tuesday...thats the day i meet with the new dr. too. big week next week. the vites should be in.
