R N Y to DS (Including Distal)

Hello:

Has anyone had the revision from RNY to DS?  I am hoping to get some feedback -hospital stay, recover, vitamian deficiency  and weight loss results.  I am in the midst of starting the process and trying to get all the info I can get.  :)

I went from 400 to 200 now I am 270.  Had RNY in 2008 with Duke.

I am an RNY to DS revision.  I was in the hospital 5 days.  My vitamin deficiency is low D & K, I am working on getting that corrected.

Who is your revision doc?

HI, Dr. Sudan at Duke will be doing the revision.

I had my psy eval yesterday and need to meet with her again on 11/12 for insurance purposes.  Did I read your timeline correct that you started the process in 2009 and didn't get approved until this year?  Do you consider the revision a success?

Yes, nearly 3 years battling with insurance.  But I WON!!!!!!!!!!!  I did have help, from people on this board, and proboards as well.

I do consider the revision a success.  I AM NOT DONE YET!!!!!!!!!!!!

 

Less than 50 pounds to a normal BMI.  I have lost over 50% EWL, and I am HAPPIER!!!!!!!!

What do you mean by "including distal"? Revising a proximal RNY to a DS is an entirely different thing from revising a proximal RNY to a distal RNY. Don't let ANYONE tell you're a DS and a distal RNY are "just the same", 'cause they ain't. (*grin*)

I have been confused over the last couple of months.  My understanding was the DS included the shorting of the bowel which alone is called the Distal bypass - revision.

Here's a pic of an RNY:

This is NOT to scale---in reality, the 'Common Limb' is actually most of the small bowel.

Here's a pic of the DS:

(This isn't really to scale either, but it's a lot closer.)

As you can see, the stomach portion of the two surgeries is VERY different. This requires stitching the pouch and the remnant stomach back together, then performing a sleeve gastrectomy. The small bowel is also sewn back into its original shape, then the actual 'Switch' is done.

When revising a proximal (the 'average' version) RNY to a diatal, all that's done is to move the 'Y' connection farther down, making the 'common limb' shorter. This means you have more malabsorption, but you still have the pouch/stoma combo. Basically, the distal, or ERNY, gives you the worst parts of both procedures.

Make SURE whoever does your revision intends to restore your pylorus---only then will you have a 'real' DS.

Revision from proximal RNY to distal RNY is not the samt thing as a revision from an RNY to a DS.  An Rny (whether proximal or distal) always involves a stomach pouch with an intestinal bypass.  The DS always involves a sleeved stomach with an intestinal bypass.

Does Sudan even do the RNY to DS revision?
