Want info on the Switch, had RNY in 04

Hi all I'm a long time member here. In Aug 04 I had the RNY. It was a successful failure. i lost weight but now 8 yrs out I'm at or near my pre surgical weight. i failed my surgery and it failed me. I had a really rough time with the RNY but am confident that every surgery is a major procedure and sometimes bad things happen. 

I'm looking to hear from you all about your experience, your recommendations for prior to surgery, just after the switch and the years to follow. My research has lead me to believe that the DS has a better success rate for super morbidly obese individuals. 

I'm no longer beating myself up over the previous successful failure, I'm looking forward and have my boys to live for and my fears of leaving them are present daily at my current weight. (398 or there abouts) I'm not giving up. I have to do this for me first and then for my husband and boys.

Also if you had Dr Vanguri for your revision I really want to hear from you. He did my sisters Open rNy in 2000 and she is doing so good with it. I love him!!

Hello Susan!  I am a RNY-DS revision patient myself.  I had my original RNY in May of 2008 and was revised to DS in April of 2011.  You should know upfront that there are only a handful of surgeons in the US that can do this complicated revision.  Just accept it.  Not any DS surgeon can do this.  Don't let anyone else tell you they can.  These are the ONLY surgeons you should consider:


Dr. Rabkin, Dr. Keshishian, both are located in California

Dr. Elariny, located in Virginia

Dr. Roslin, located in New York

Dr. Buchwald, located in Minnesota


Of course, the above doctors are in the United States


Dr. Gagner, located in Canada (Does most revisions in two parts)

Dr. Baltasar, located in Spain

Dr. Marchesini, located  in Brazil

Most people have to travel to get a RNY to DS revision, but it can be done!  Good luck!


A lot of the vets that have a great wealth of information are somewhere else now.  So, I would highly recommend that you join the other site as well.  You can get info both places.  Check out my siggy for the link!

I will add that I would go with Rabkin, since he is the one who gave me back MY life!

Oh, and your RNY failed you. You did not fail it. By design, it is destined to fail for most people.

I don't know whether or not Dr. Vanguri does RNY to DS revisions - hopefully someone else will know, or you will need to contact his office and ask. I haven't heard of him as  a RNY to DS revision surgeon, and we tend to know these things around here.  Be very specific about what type of revision you want, or you will be told that OF COURSE! he does revisions, you'll get there are find out it's just "fixing" your RNY, or lap band to DS revision, and you'll be very disappointed.
    And on that subject, Dr. Marchesini has a stellar reputation, but prefers to revise RNY is a way that keeps the old pouch, rather than revise to a real DS. He feels this is safer. He's probably right, but if you have any pouch problems, it won't fix them. However, my understanding is that if you insist on a real DS, he'll do it.

I hated my RNY--nearly shat myself most days with that horrible thing. I'm not even talking about weight loss.
Living WITH a pyloric valve is much better than without.
It's a hard road to revision. Took me five years to get to revision surgery. Few surgeons, and surgeons are picky.
Insurance won't want to pay for a revision; most insurance will say "one surgery per lifetime" or "patient messed up." Patient messed up is not true; a surgical stoma is not as good as the natural valve.

Hi Susan,

You didn't fail your RNY, it failed you.  I had RNY in December 2004 and initially I did well until the 6 year mark when I gained weight then I had a baby in 2009 and it all went downhill.  I had a revision to DS in September and I am doing great and have lost 50lbs so far, my surgeon is Dr. Roslin.  Please research, read www.dsfacts.com and ask questions on this site and revision board.

There are only two docs I personally would go to for a revision after reading revision (RnY to DS) stories here for almost 3 years and they are both in CA.  Rabkin or Keshishian.

There are a handful of other surgeons (literally very very few) but if it were ME, I would chose from those two.

Dr. Keshishian did my RNY to DS revision 12/10. He or Dr. Rabkin are the ones to go with. Travel is almost a given.
