This is the info listed on DSfacts.com.  It didnt have a phone number listed, but it did include his email.  There might be someone who has had recent surgery with Dr. Gagner at the site listed in my signature that you might be able to ask as well.

Email is the best way to reach him. He always answers promptly. The one above is correct.
