Drink some protein shakes and don't worry about the calories. Count carbs to keep under 50 grams a day. No idea if you can talk sense to myfitnesspal and change it to work that way.

Atkins Ready to Drinks (Walmart) are yummy.  They also have Muscle Milk which are good too.

Get your protein UP before surgery!!!

 I love MyfitnessPal.  I use it everyday to track my carbs and Protein.  You can customize it to fit your needs.  In SETTINGS you can change WHAT you track. and in GOALS you can customize how many grams by percentage you want of each.  
I track  Calories (required)2000 easiest number to get to my desired  Prot. and Carb goals.
              Protein  125 g at least
              Carbs       50g
              Fiber         15
              Fat.             don't REALLy care, just there.
