Do you have a surgery date?  If you have a couple of months you could start taking dry D3 in larger doses to bring your D up. You might want to try 50,000IUs every other day and retest after about 6 weeks. Or take 20k every day. You can get dry D3 at VitaminShoppe in 5,000IUs capsules.

You might also start taking a 100mg B-complex now to bring those levels up. What was your B12? If you're not at the top of the range, you might want to start taking a sublingual B12 every day. Do you have a vitamin A or K level?

If you only have 3 weeks, that large a dose of dry D3 is fine. You aren't going to go toxic or anything. Take it every other day if you like.

The B-complex is fine to take now. And, get going on that B12, people can be symptomatic at 400. You want your B12 up and over the top of the range.

I would start the dry A and K1 every other day now too.
