Do you know of a link or maybe where I could go to find an information card to explain the DS surgery just in case I have to go to the emergency room one day or something I can use to explain my surgery to my PCP when that time comes? Any information I will greatly appreciate it.

My doctor had given me something like this but I sure like this one better - I like having it filled out and handy. I have worried about this in the past - thanks for sharing this great version!
