There are fewer than 50 surgeons in the US qualified to do the DS sufficiently to make that list - that is FAR fewer than the number of surgeons who say they do it (but you will be told you don't qualify for the surgery once they get you in their office - that is called "bait-and-don't-switch"). You are going to have to travel - and since you are going to have to travel anyway, I would suggest that you travel far enough to use the very BEST surgeon, not necessarily the one closest to you.
