Again not sure where you get your info from but you are wrong. I have been to my two local hospitals plus a small ER/Urgent care where I live.

I have been seen at one hospital that Dr E practices at which is Fairfax Hospital and the last time I was at his office was a little over 1 week post op from my original surgery and he didnt want to see me again until 6 months post op due to the plan we discused with him and my PCP and specialists exchanging information etc.

Why they are not sending my records I have no idea. All I do know if I have signed many consent forms for the release of my records and they have been faxed, mailed, emailed to both Dr E's office and Fairfax Hospital. Why I havent gotten my records or any of my Drs who have been requesting them is beyond me.

They may not have the proper release, the request may be going to the wrong department, the information being requested may have the wrong identifiers...  It could be a MILLION things.

As long as it has her proper name, date of birth, SS#, a statement of what is requested, and who it is to go to, you can even use a template, there is not such a thing as a proper format.  There are no wrong identifiers...you put either "all records and notes" or define what is needed for a particular condition. Or put "all notes and visits after a certain date"

Office even has a template for one and once edited, I have even used it to obtain my medical records for another doctor. I keep it in my computer for those times I don't have time or want to waste time waiting for a form to be mailed to me.

Sounds fishy to me that even after her medical doctors have requested those same records be sent to them that they still haven't been released. It's almost as if his office is holding them hostage til she finishes paying for her surgery.

And releasing medical records from a hospital should be a routine matter...as long as it is going to the Medical Records dept with the same info outlined, they have no option but to comply.

This whole "not releasing records" sounds like she has a leg to stand on as far as that goes.

You are going to tell me my PCP, Liver specialist, GYN, Digestive Health Drs and other people requesting my medical records are ALL doing it wrong? They had me sign the same medical waiver that I have signed many times for release of my records. Hell it was just about the same release that I had to sign at Fairfax for them to get my medical records from the ER's here.

I have also contacted Dr E's office and the hospital and have requested them in writing, over the phone, via email and via fax and still have not gotten them.

You mean to tell me that everyone is doing something wrong except for Dr E's office?

 Then go in person...  OR-

To get records from Dr.  E, speak to CURTIS, not Alex, or anyone else.  Email won't get you a response, faxes sit on the machine.  I've told you this, multiple times.

Here is the info for medical info release from INOVA.

I dont remember you ever telling me this unless you said it on the board.

I am supposed to make a 4 hour round trip without traffic in pain to get my medical records that have been requested by myself and many of my Drs because they cannot fax them like they are supposed to?

I have spoken to Curtis and he transfers me to Alex as he says she handles everything. I cannot understand Alex on the phone as I dont hear well and she has then asked me to email her and I do that and she doesnt email me back. I have tried everything in my power to get my records. I really dont think it should be this difficult to get my personal records. Faxes, calls and emails should get responded to and returned especially when they asked me to email as they are aware of my hearing issues.

Yep.  I've done it.  If it's that necessary to your care, and all the proper avenues truly aren't working, then yep, that's what you have to do.  Make your husband, mom, friend, neighbor, taxi, whoever drive you and get it done.

Kimberly, I find that when a doctor's staff member is doing this, asking to be emailed and then not responding, that adding the doctor's email if you know it to the next one works wonders. All of a sudden they know 1) you mean business and 2) their boss knows they aren't complying.

And regardless of who you faxed/mailed the release to in the office, you should have them. Whoever sorts the office mail in the doctor's/hospital's office should route it to the proper dept/person without you handholding them.

I'm sure hysteria is involved. When you are that sick and are trying to get to the bottom of it... you bet your hysterical. If I was her I would have had it by now. I would march right to that doctors office and stay right there why I waited for them to photocopy all my records. She really could be having problems getting information from the doctor. Everyone has different experiences. You have to stay on top of your doctors. They are so busy and have so many patients. I always have questions and open my mouth if I need something or disagree with something. No one knows how you are feeling but you.

I know when I was so sick for months, I was in a bit of a state of anxiety because I was anxious about being sick and wanting to just get better. I know I could get impatient waiting for doctors appointments. Some take a while to get into. I think you really have to advocate for yourself and keep looking for the answers. I would not give up. I kept going from my PCP and calling my surgeon and having tests done. The bottom line is she just wants to get better.

I suppose then that it is her job to KNOW who that magical records person is. Ya right! I know all the magical record people wherever I go. It is sort of like a sixth sense.

No, but when at least two people have told her, a few times, who to speak & deal with within the practice, she should be well aware.
