Rabkin does NOT charge a program fee. However, he does not participate in any insurance plans, so he gets his full fee (although he operates at hospitals that participate) no matter what. To ensure that, he requires 2/3 of his surgeon's fees upfront before surgery. If the insurance company pays more than the other 1/3, you get the amount they pay over 1/3 refunded. If the insurance pays all of his fee (unlikely), you get it all back. I believe the 2/3 is about $9K now.
As an example, my insurance (Aetna) paid $7500 of Rabkin's at-that-time $12K fee. I had paid $8K upfront, and I got $3500 back from Rabkin. So I was out-of-pocket $4500 for my surgery -- well worth it. The hospital charge was $700 -- $100 copay, plus $200/night for a private room (so my husband could stay with me).
