DS vs. RNY?  The clinic that I plan to use (Duke Health, for those of you in NC) only has one physician who does DS.  He was the one who gave our presentation for the pre-app meeting that I attended.  It seems that DS is more effective than RNY, causes different vitamin malabsorbtion, doesn't cause dumping, but causes horrible wallpaper-peeling gas.  Am I on the right track?

DS as a procedure - I seem to remember DS can be done as a lap procedure, but takes much longer than the 90 mins of the RNY lap.  Am I correct about that, and how did your actual surgery go?

Are the post-op complications different for DS?

Is the gas really that bad?  Seriously?  Is there a trade off where you don't get constipation and need constant magnesium supps like the RNYers?

If I'm going to do this thing, I want to do it right from the beginning, which means allowing all of my options to be considered.  I'll allow my MD to give me a goal weight, but I believe getting to a normal BMI will require about a 170 pound loss.  I know DS is more effective than RNY.

What resources did you use to make the decision to DS instead of RNY?

IF I eat a giant bowl of ice cream, no lactaid, with cookies and whipped cream and all kinds of nonsense on top. YES the gas can be that bad, well I mean it's different it smells different so it seems worse to people's noses because I still have a kid in diapers, okay, and when you can smell the chubby monster from across the house around the corner NOW THAT is bad.

Day to day, I don't much have a single fart though because I eat low carb or complex carb if you will (we DSers absorb complex carbs at about the same rate as protein, so when you eat whole wheat as a DSer you don't absorb all of it).

I think as far as constipation goes ymmv. But in GENERAL I think DSers pretty much keep up on getting plenty of fat in their diets and stuff keeps on keeping on.

The board is basically what I used to make my decision, reading all the articles posted. Not only was I super heavy but I was much younger and I wanted BOTH the best weight loss and the best chance at keeping it off long term. Seeing stories of DSers who just cut sugar out of their diet and drop 8lbs in 2 weeks YEARS out made me hopeful that with simple adjustment I could finally CONTROL my weight without having to fight my own body's built in defense against starvation mechanisms.

I have no issues with gas.  Let me say that again -- I HAVE NO ISSUES WITH GAS.

This is one of the myths that follow the DS around.  I will not say that no one has gas issues but I will say -- not everyone has gas issues.  I get no more gas than I do as a pre-op and it does not smell any worse.

The difference in time on the table should not really be a factor.  If operative time is a factor due to some pre-existing condition then your surgeon would let you know this.  My DS was open so I haven't a clue about the lap option.

I'm still a pre-op, so I can't answer all your questions. But I will share why I chose
DS over RNY (after a lot of research and pondering)

1) DSers have a full working stomach and retain the pyloric valve. That means that the stomach can be scoped and there's not a blind stomach in there waiting to get sick.

2) Because they have a pyloric valve, DSers can drink and eat together. (Eventually, at first liquid takes up a lot of room and you need that valuable space for protein.)

3) The DS diet is one that I see myself being able to follow long-term. If I was able to eat low-fat all the time, I wouldn't need surgery!

4) The statistics show that the DS is the one with the most success over the long term.

5) 98% rate of success in getting rid of diabetes!

6) This is the surgery that people revise to!

7) The peeps on the board are much more fun to hang out with!! I can't wait to be on the dark side.

Hope that helps some with your decision. I'm sure some of the vets will chime in too.

Well sort of; the malabsorption is why we lose so much better and have such better long term results, In general we only absorb 20% of the fats we eat, 50% of the protein and 50-60% of the complex carbs we eat. We get no malabsorption help with simple carbs or sugars, so we have to watch those, if we want to lose or maintain.

The gas business is a bunch of Bull **** You fart now and you're going to fart after you have WLS, it's a normal bodily function. With the DS, the amount of gas you create is easily controlled by how many carbs you eat; if you eat excessive simple carbs, you will have more gas, it's really that simple. The smell can be bad, just like it can before the DS or with RNY.

DS as a procedure - I seem to remember DS can be done as a lap procedure, but takes much longer than the 90 mins of the RNY lap.  Am I correct about that, and how did your actual surgery go?

Yes, the DS can be done LAP and it usually takes 2-3 hours for a virgin DS; my revision from RNY was a lot more difficult; took two surgeries the first one took about 8 1/2 hours, the second one was three days later and it took about 4 1/2 hours.

Are the post-op complications different for DS

Not really, it's pretty much the same.

Is the gas really that bad?  Seriously?  Is there a trade off where you don't get constipation and need constant magnesium supps like the RNYers?

What resources did you use to make the decision to DS instead of RNY.

You're 100% correct, the DS is far more effective for helping you lose and maintain your weight. It's also much easier to live with than RNY, as in how you eat every day. With RNY, you will be on a forced diet for the rest of your life if you want to lose or maintain. With the DS, you lose and maintain eating like a naturally thin person.

I used my 28 years of experience living with RNY to help me make the decision. I knew I didn't want anything more to do with that damn pouch, I was sick to death of the dumping and food sticking and not being able to lose the excess weight.

I love, love, love my DS!

Continue to read this board and gather all the facts as you are doing.
Having the DS instead of the RYN was the best decision I've ever made in my life!!!
Gas can be an issue but not always. It is a personal thing as to what you can and cannot eat after surgery (and well beyond). Some people have problems with lactose and or carbs. I was lactose intolerant BEFORE surgery but now I don't have too many problems with it. Soft cheeses like goat and cream cheese gas me up and cause oil slicks big time, but harder more aged cheeses such as cheddar don't bother me at all and I eat them everyday!!!
I live in a household full of men and my gas is now legendary! I am a tad bit proud of the fact! But it is controllable and should not be an issue that you can not deal with.
Good Luck!
