Emergency Medical Card

Is there some place I can order a DS Medical Emergency Card. I thought I had seen a generic one online somewhere but can't find it now. I have found a couple that have the surgeon's name on the card but was hoping to find one I could use that I can put my own surgeon's information on it.

Hi Charlie,  I have one on my profile, just a month or two back... I printed it out and keep it in my wallet, u could laminate it too.  My family knows it's in there.

Dr. Rabkin has one on his PacLap website that can be downloaded and printed out.     

Someone posted this one awhile back.  I printed, trimmed, used double sided tape to put together and then laminated.  Hope this will work for you!
