DS Doctor who accepts Medicare?

Anthone in Nebraska, and I believe Buchwald in Minnesota.
Gagner used to, but I'm not sure about how his transition is going, or if he's able to accept Medicare in the new facility (has to be deemed a "Center of Excellence" by Medicare first).
Any Gagner patients out there with more specifics for Lisa?
I believe Anthone and Buchwald would be open DS, and Gagner might be a two-step, laparascopic progression (likes to perform gastric sleeve part first, then after weight has been lost, the actual "switch" part performed later.
