Topic: RE: type 1 - wls - enough carbs to avoid DKA
Hey Mishelle,
The answer is unfortunately going to be different for each person with type 1. I had gastric bypass back in 2004 and got really sick afterwards because of DKA even though my blood sugars were fine, (never higher than 110)!

Anyway, once my doctor's and I figured out what was  happening I left the hospital with the instructions to take at least thirty units of Humalog a day, (in addition to the 12 units I get from my pump basil rates), so that added up to 42 units/day.

For me, that was eating/drinking ~200 carbs a day.  Needless to say I drank a lot of 1/2 water 1/2 apple juice drinks just to get to that!

It's really important for you to work with your doctor's to make sure you'll be all right and ready to go after surgery. Only eating 5 grams of carbs a day for a person with type 1 is unrealistic and can kill you. I was really sick, almost dead after surgery so trust me I know.

Let me know if you have any other questions!

Topic: RE: Discouraged
What are you eating and/or drinking? Maybe you have some type of infection? Maybe its just insulin resistance and you need a different type? What has your endocrinologist suggested? How about a thyroid test? I know my sugars were totatlly whacky when my thyroid was off.

I'm so very discouraged.  I had surgery July 22nd and my diabetes is worse than ever.  Pre surgery I was on Lantus and Actos and my BS's were all within relative normal range . . . A1C was 7.0.  Now I've been running in the high 300's and am on Lantus, Humulog, and Actos.  Nothing is bringing it down past the 200's.  I'm discouraged, angry, and scared.

And, since someone (a VSG patient) tried to argue with me recently that HOW LONG you've had diabetes has nothing to do with whether or not you may experience resolution after WLS...

Effect of sleeve gastrectomy on patients with diabetes mellitus.

Rosenthal R, Li X, Samuel S, Martinez P, Zheng C.

Bariatric and Metabolic Institute, Department of Minimally Invasive Surgery, Cleveland Clinic Florida, Weston, Florida 33331, USA. [email protected]

BACKGROUND: Limited data are available evaluating the effect of sleeve gastrectomy on the control of DM. Our objective was to evaluate the effectiveness of sleeve gastrectomy in improving glycemic control of obese patients with diabetes mellitus (DM) at a research clinic. METHODS: A retrospective review of a prospectively maintained database collected between January 2005 and January 2007 was conducted. A total of 30 patients with DM who had undergone laparoscopic sleeve gastrectomy were studied. At the 2- and 6-month follow-up visits, we analyzed the variation in glycosylated hemoglobin, fasting blood glucose, and body mass index. RESULTS: Of the 30 patients with > or =6 months postoperative follow-up, 22 (73%) had been taking medications for type 2 DM preoperatively. Resolution of DM was observed in 27% at 2 months and 63% at 6 months of follow-up. Glycosylated hemoglobin decreased from 6.36 +/- .82 (n = 14) preoperatively to 6.02 +/- .57 (n = 11) at 2 months and 5.92 +/- .33, (n = 12) at 6 months after surgery. The body mass index decreased from 46.12 +/- 10.86 (n = 30) preoperatively to 38.27 +/- 6.59 (n = 30) at 2 months and 35.78 +/- 5.11 (n = 29) at 6 months after surgery. Patients with a shorter duration of DM (<5 yr) and better weight loss after surgery achieved greater resolution rates. CONCLUSION: The improvement and resolution of DM in obese patients has been observed as a result of weight loss after sleeve gastrectomy.

Never been on a pump, been type 1 since I was 18 years old. I take Novalog & Levimer before surgery I was taking about 175 cc a day, now i take roughly 30 cc a day. I had surgery in May this year and so far have lost 94 pounds.

But you really need to get your blood glucoses addressed.  BS in the 500s for more than one day is very dangerous.  If you do not have an endocrinologists you can call and get in to see right away, otherwise go the emergency room.  You need to get this taken care of now~
Thanks for reading!

dear frustrated pt,
As I told you before you need to have a serious sit down with dr. sapala and mary about you, your health and your surgery goals. You should be on an atkins diet as per mary zotos the nutritionist. the purpose of this is to regulate your glucose g1 levels. I too was a diabetic and their advice has me know way under control. You need to see him asap dont delay. 500 plus is very dangerous and bad starches like bread, white rice and flour can be very harmful. Try meat and green vegetables. Ask Mary z to give you the list of what to eat and how often. Good luck and God bless.
