not a doctor but i reduced my basals by 20% pre op diet - and 40% post op - but i'm type 1 and on an insulin pump. Highly suggest you get your endo on board, they will need the consult when your in for surgery and post op as well!

Jen, my NUT said we probably wouldn't go hypo. I haven't had my surgery yet (Mon.) so I can't tell you first hand. That is just what she told me.

My doc had me take all my meds up to the day of surgery. On the day, they reduced the insulin to half, no Byeta and no metformin. My liquid diet is only for one day, though. Regular eating til Sunday, then liquid... plus laxative times 2. :}

Hi folks,
My application for the sleeve has been rejected, so I'm off to Mexico for April 1. My BMI is 37.4, so I'm required to do a 7 day liquid diet, pre-op.
My only challenge is that I'm not getting a lot of direction from the surgeon's office about how to reduce my insulin needs during this phase. My PCP doesn't feel that she is best placed to give me that direction, and it is difficult to get into my endo at short notice. I'm trying to find a bariatric doctor in my area that might be able to help, but there are certain ones in my city that I refused to deal with, because of integrity issues (on their part).
In the meantime, I was wondering if anyone could share their experiences on how they managed their insulin (background) and metformin, during a liquid diet phase.

I'm 7mos post op, and been on the pump for 12 years. When I was newly post op I changed my target sugar range to be higher than I am now. SSay 150 vs 100, that helped me avoid the the lows considerably when iw as first out, and really not able to handle anything to eat much.
i was using 130-140 TDD pre RNY and now I use about 40-to 45 TDD - type 1 diabetic for almost 25 years (one month from today)
I do use the glucose tabs, sometimes the liquid glucose bottles if really low (was 56 the other day) Also milk gets into the system fast, and works great to bring up low blood sugars and sometimes for an added boost will add some NSA carnation instant breakfast. A little more protein and a little more carb to raise sugars as needed.
I have not dumped on sugar - and you do what you have to.

I used it for about 9 months - it worked great for me for both my weight and my blood sugars. Unfortunately, it turns out I was smuggling it in to Canada. I declared it every time I crossed the border but the last time, the guy was really mad. He let me go so I decided to check into it. Nope, not allowed in Canada without going through bureaucracy.

You should ask this on the RNY and/or Main Board. It is not at all uncommon after RNY.

Most insurance companies will not recognize insulin resistance as a comorbidity, only actual diabetes. Depending on your situation, you may want to switch to a doctor who doesn't believe in "pre-diabetes." Some will diagnose pre-diabetes, while others will just say you're diabetic because you can't be "almost" diabetic (like being just "a little bit" pregnant - it doesn't work that way!).

Because you're going for a VSG? They turned you down for an uncovered procedure, not for your lack of comorbidities. BTW, have you looked into the DS? It has the highest type 2 diabetes resolution rate of any WLS procedure.
