Type I on Pump - lapband vs gastric bypass

Hi all,

Can you give me some advice...I'm in the research part of weight loss surgery.   My endo asked me to look into it, so here I am.   She wants me to think about getting a lapband, however the seminar I went to made it sound like gastric was really the only way for diabetics to go.  

Has anyone out there on a pump had either the lapband or gastric bypass.   Which one?   How have your BG's been after the surgery?  how was the healing process.

Any advice you can give me would be great,

I orginaly was going in for the lapband. I didn't want the RNY. But then after looking at my options I discussed things with my PCP. I valued his opinion on this matter. He had a discussion with my surgeon to figure out the best choice for me.

Typically speaking 80% of successful lapband patients loose 60 pounds in the first year. That's a lot of weight, but not the 125 I lost since I had the RNY done. My PCP said I should pick the surgery that is going to allow/help me be the most successful. My BMI was 57 to start with and through research my PCP did as well found that complications worsen with a bmi higher than 43. (I don't have the articles to back it up, he is a doctor that has had band patients and also saw their success) MY Surgeon was nto selling on one or the other, he was goign tos upport me no matter which one I had.

I decided to go with the RNY. I"m so glad that I did. right away after surgery i regained my insulin sensitivity. And a year out I'm using about 45*-50 units a day vs the 130-140 I was useing last year.

WIth what my insides look like, a lapband would not of worked for me. I've been diabetic too long.

Hey there!

welcome to the world of options!

it really goes down to how much you need to get off, how you eat, what you are willing to do, and what you insurance will do.

for me i was sold on RNY. i was ready for the change.. i had 100 lbs to loose. and my doc even told me it would be good. Then i went to my insurance and they said no i only weight 95lbs over weight and they were not approving it. ..... soooo yah i got those 5lbs on and went back in may to my doc and re did everything.. however this time around we talked more about the band.... i would be able to treat a low and not worry about dumping, i would have to do more work with it but later on if i start gaining they can tighten it up and ill be loosing again.. so after some time i choose the band... and so far so good. I'm already off two medications and its been 3 wks since my surgery. i am down 20lbs and a pant size. Ive had some complications but that was because i had a hiatal hernia repair with it and it was very severe my doc said sooo with everything i am still very happy with my choice. i do wish the lbs would come off quicker like with the rny.. but that's why i get on my treadmill everyday and just go into a zone for 40 mins...

the biggest thing is to do what is right for YOU! you are going to have to deal YOU are going to have this done to you. A lot of people have opinions on what worked for THEM but you aren't them.. do what is good for you and everything will work out.

as for my BS after surgery have been IDEAL! nothing over 200.. mainly because of the diet. I'm on 50% less insulin its great! treating a low is a pain( i have a hard time swallowing).. but only because of the hernia repair not because of the band... the healing process was quick.. i was walking around walmart the day after surgery lol... (not by choice) but i was... keep walking after the surgery it really does help with the pain and discomfort!

Get your doctor to put you on the new drug Victoza, it has a wonderful side effect of weight loss. I have been on it for 13 days today and have lost 9 lbs. I have had the sleeve and lapband and neither one worked for me. It is for type 1 and 2 diabetics but it is so good at weight loss that some docs are putting their non diabetic patients on it in smaller doses. I think I have found the miracle drug. Good Luck Judy

for me milk works great for treating a low. And if needed more omph, I will add carnation instant breakfast NSA.  I'll drink half see how I'm doing and do the other half. If milk is an issue, then use something else lactose free.

Not dumped to treat a LOW, or dumped ever and I'm a year out btw. Though not all the same.

I myself was really worried about being able to treat a low after surgery, fear of dumping and all. But what was great was being able to increase my target BG on my pump for a month or so after surgery, change insulin sensitivity along with IOB feature.

Thanks for the insight. I also am Type 1 on a pump and treating lows is part of the worries. I have discussed this with my PCP as well as my NP and they both reassure me that it is managable. So time will tell. My RNY is scheduled for Sept 28. I am anxious to get this onto the next phase.

BTW, for those who have had the procedure, what has been your post-op recovery? I work part-time and wondered about returning after my 2 week office visit.

Hi Jennie,

I had lap band surgery 9/24/09 and I am now one year later at the exact same weight. I wear a pump and have been a type 1 for over 30 years.  I don't really eat anything bad or considered bad for this diet.  Sure, I slip occasionally, but not enough to make the whole thing  a mess.

The band is only restriction....meaning you can't physically eat that much at one time, but if desired you could eat all the time, just small amounts.  I find that if I eat only cottage cheese, protein shakes, yogurt and oatmeal, and exercise....I lose weight, probably 3-4 pounds a week, but the minute I add regular food back into my diet, beef, pork, chicken, a couple of carbs....crackers to eat the hummus on, I gain weight. Instantly! I think it is the insulin and its just turning everything right back to fat.

With the band, I struggle to eat hamburger meat, ground turkey, anything like that...pork chop. I usually stick to hummus and crackers...about 6 per meal, maybe less. I do like yogurt, so at least there is that. I use J. Robb protein or Designer Whey protein with non-fat milk and low-fat peanut butter or sometimes half a bananna.  The protein shakes help a lot.

My sugars are in good control and always have been. The band just does not work for me at all. I am working on approval for revision to RNY. I chose the band in the first place because it was reversable. I don't know why, but in my mind...that made it better! It wasn't for me. Also this time around, I am hearing from the same doctors that this will be much better for my diabetes because of the malabsorption part of RNY. I wish someone told me that a year ago!!!  Still, I made my choice and have lived with it for a year.

Its been a year of throwing up, going for adjustments, throwing up some more, even though I'm eating what I am supposed to, feeling hungry and basically not getting what I wanted out of it at all.  Everyone that I know that has had WLS...people that I see in person...the ones that have had RNY look fantastic.  The banders on the other hand...some look great, but most do not. I've given up on the band and am crossing over to RNY.

I healed very quickly after surgery. No pain to speak of. It was done laproscopicly. The only pain that I felt at all really was at the port site....because the realize band goes into muscle, so that was sore, but after about two weeks, it went away. The first few days after surgery...I took benadryl at night to just nock me out.....actually, liquid tylenol pm...but it was so sweet. After RNY, I don't think I will be able to do that, but I am sure I can crush the benadryl tablets into some warm tea and drink them.  All you really need is a little help getting to sleep because for me, I had to sleep in a different position for a couple of weeks to give everything a chance to heal up.
