If an insurance policy covers bariatric surgery, they generally require a BMI over 40 or BMI of 35-40 with comorbidities.

Hello

Here in Canada... There is a criteria...

It appears that you would qualify.

Sleep paralysis ? Do mean sleep apnea?

BMI 45 yes... Hypothyroidism I am not sure ?

Do you have a history of depression?

OR are you currently depressed?

I know for a fact that if you are depressed?

You need to be actively treated for depression.

There is a long process for preparing for gastric bypass surgery.  You would meet with the weight management doctor, nurse practioner who does a complete and thorough medical history.  Here in Canada you attend information seminars that educate you through the journey.  You meet with dietician &  behaviourist.  Pre & post op classes. There is a significant risk for depression post operatively.  They really make sure that you are ready both physically and emotionally.  Then and only then do you get the green light for surgery....

Good luck

Mahalo

A BMI of 45 would qualify for any of the surgeries (over 40, or over 35 with co-morbs).
