Have you already chosen your surgeon? Mine contacted my insurance for me to find out what criteria I had to meet (I had to do the six months supervised diet and have 5 years of documentation of being obese).

ETA- If covered, most insurance companies require a BMI over 40 or 35 with comorbidities.

I am 5'11" and started with a 53.4 BMI. Once you are over a 40 BMI co-morbidities usually aren't required. Being morbidly obese is a morbidity in itself. As Laura said each insurance has different requirements. I spoke with my insurance company and they emailed me a link to their policy. It also has to be covered under your insurance. Some companies do exclude it in their policy.
