Going from a 74 bmi to 43 is pretty impressive for just having a sleeve! Good work!
There are lots of folks that have had the sleeve then the DS. It used to be done in 2 parts for super high BMIs by a few surgeons, but now, most competent surgeons that do the DS now to do it in one stage.
Most that are doing it in 2 parts, are those that had sleeve, didn't lose as much as they wanted or are having trouble maintaining or even regaining, and want the help of malabsorbtion. There are lots of these folks on the various fb groups. Some groups are private or closed, and some are more informative, some are just cheery support, so message me if you need some help or info.
You may already know, but revisions generally lose more slowly than doing the DS in one surgery. There is a synergy that happens when both the sleeve and bypass parts are done together, and many people have increased metabolic issues that may have been worsened with the separate surgeries. Still, revising to DS is your best shot for a successful revision, and it should help you get much closer to your goal.
I'm glad your looking at the logical revision to DS, as I keep seeing sleeves being butchered into RNY, which is a sign of an unskilled surgeon, and the patient ends up with an inferior surgery because of it, as well as all the extra trauma to their stomach. I suspect you've already done a fair amount of research since you know DS is what you need. Hope to see more of you around.
Definitely check the DS board. Also make sure you get a surgeon who is skilled with revisions. I would agree to never go RNY after having the VSG unless you have no other choice.
I went from a BMI of 79 to 54 with the sleeve (299 today, down from over 430 last June). If I do revise, it will only ever be to the DS.
I follow a ketogenic diet post-op. I also have a diagnosis of binge eating disorder. Feel free to ask me about either!

You have too! That's very exciting! Good luck:)

Dang, that's impressive. I was sleeved in 2009 and went from 389 pounds to 350 pounds. Not a resounding success. Now that I finally have insurance that will cover a revision to DS, I am going to see a surgeon. I will be interested to see what his insight is into why my sleeve didn't work. Yours sure worked, though. Congratulations on your achievement!
