waist training?
I second the comment, just google waist training and look at the images. My friend, a goth model, got her waist down to 12 inches. That is obviously quite extreme. But to address how you get one, you need a corset. They are generally quite expensive and hard to come by for people with BMI over 50

I am not waist training per se -- but I currently have a hernia and the doctor recommended for me to get a waist cincher until the surgeon can see me in August. I ordered it through Amazon.
I will say that it does make a difference. My waist has gone from 29" to 28" in less than a day and a half -- even when it's off. It's really weird to me!
