Anyone have any info on Dr Levy in Toronto??

Hi everyone.  I have been a lurker for a while, but finally decided to break my silence lol.  My doctor filled out the form to be sent to the Ontario Bariatric Registry after talking to me about WLS due to my BMI being 79.9, this was about 2 weeks ago and I just received a call from the office of "Dr L Levy at 174 Duplex Ave in Toronto" asking me to come in and see him.  So I am going on March 3rd.  I googled the Dr and the address and couldn't come up with much information about him other then his practice is located right in a bunch of housing area in Toronto.  Has anyone else ever seen this guy before?  If so....what is going to happen at this visit?  I thought that I would of been going to a Bariatric Center but I guess I am kinda confused about this whole process.
