I am a horder so I had my card from WW as proof because it showed weight and each time I went.

I kept my weigh-in card. They will not (or atlesast they didn't) give print outs of your weigh-in's but that may have changed.

I see my PCP on a regular basis so she had my weight history.

It may be an option for you.

I made a copy of my weigh in card. Since it had the dates and weights of all of my visits, this was all the proof I needed for the six months visits. With BCBS (CAREFIRST) this was sufficient.

Great! HS, did you have to submit any additional letters from other physicians other than your surgeon? (PCP for example ...)
