Could it be acute indigestion?
I have had a few bouts (4 or 5 episodes) of it so bad that it radiated through my mid-back, much like what you describe. I took a couple tums/rolaids and did some stretching (using a door frame to press my back against) to get relief. It felt at the start more like the pain was muscular, but pain can 'refer' to another location like that.
If this doesnt resolve quickly, call/go see your doc.

When I've got that choking lump in the front of my neck it is generally my thyroid. It sometimes develops to swelling and warmth/ tenderness to the touch. Your levels can be within the 'normal range' even with these symptoms.
Backing off of anything with soy/ tofu and veggies like broccoli and cauliflower for a while help me.
