I work for an insurance company, one of the big ones and gastric bypass might be a cover benefit but this is where they get you it has to be medically necessary. And the Insurance company determines medically necessity.  We have to deny claims  for the treatment of obesity, insurance companies wont cover that diagnosis I get tons of claims where the surgery was approved but because how they bill we have to deny. So call your doctor office again and asked them get they submit the medically necessary forms. Good Luck......I believe the reps are correct its how your doctor submitted the forms.

Find out if your insurance is self funded. This will be important if you have to fight and go through the appeals process. You might also find out how your state handles insurance appeals. A question on your state board could be helpful. Good luck
