if you could make up a questionnaire containing 4 questions that a potental mate/fb/whatever had to fill out truthfully, which questions would be on it?

here are my questions:

1.  List all the things sexually you WON'T do. (note, if you list more than 3 things, don't even bother completing the rest of the questionnaire)

2.  Still here? Great....that means you got some freak in you & thats a good thing.  OK, the next question is

What is your credit score?

3.  How is your relationship with your mother & father?

4.  How/Why did your last 3 relationships end?

OK, boo we not gonna go there.

Very close t my parents. They are more like my brother/sister that father/mother we are extremely close. I can talk to them about every and anything.

4.  How/Why did your last 3 relationships end?
Last relationship I had ended in a divorce. Was married for 15 years but it was a blessing that it ended. So now I'm just enjoying Brenda and life in general. I am now doing MEEEEEE!!!!!!

My must knows:

1.  Personal financial statements, show me yours including credit score, net worth, and tax transcripts.  I want no surprises. 

2.  Personal health history, share your medical and mental health records with me.  Ditto, no surprises.

3.  Share with me your criminal background investigation.  No options hear, I do not deal with graduates of the penal system. 

I know it all sounds impersonal, but there is no point in making an emotional investment until he passes the basic screening. 

Last question?  Skipping sex, if he's a man he wants it and there is only one way to know if he is any good. 

4.  Damn, I need more questions, but I'll ask about your relationship with your children throughout their lives. If you can't love and nurture them, you are useless to me.

3. List the name of each person residing in the home with you (if your mother is one please list her as head of household.)

4. Are you employed and if so how long have you been on your present job.

On a separate piece of paper please list every misdemeanor and felony you have been accused of and the outcome in court. If still on parole or probation please give name and number of officer you report to.
