So she remembered  me of something that I always did with my baby when she was in the hospital( to document everything).  And when my father was sick it just came as a surprise I wasn't thinking.  But I did ask every time they gave him a meds I ask what they was giving him.  Everything was good until he went to rehab can't stay there so he never ask what was the meds he was getting.  Doing the day when we was there we would ask.  The same two every time, meet with the rehab staff today,  They was trying to tell us they he was going to be there more than the 20 days.  The nurse started reading off the meds they was giving him.  Why in the He**  they started giving him the same that put him in the hospital in the first place.  They thought we was losing our mine we HOLLER so DURN LOUD why would you'll still be giving him that med. They started giving him that med in the hospital.  They looked in there notes seeing the hospital had stopped it but for some reason and had started it back.DAMN.  Now remember I saided they report he was still weak OK.
PRAVASTATIN 80mg for someone that's 82 years old with no problem before this med. Thanks you SO BLESSING the NIGHT BEFORE i HAD LOGGED IN ON MY DAILY CALENDAR THAT I CARRY ALL THE TIME. THANK'S

We always talk about standing for our kids we have to stand for our elderly parents also.  Please always go visit are send someone to see them.  The things that I have seen in the last two weeks are something else. 

I'm about to call the insurance in on them soon.

Anyone that didn't read the first posting a few week ago a Sister in Christ that work at Rehab throw a pillow at my dad because that they had took all his pillow.And wanted him to roll over OK why is he in rehab.  Then she stated she was no man, well give up the freaking job.  When I saided a sister in Christ this sister girl goes to the same church I attend.  She someone that joined after  my parent started not being able to attend church. SHE'LL WELL REMEMBER ME FOR SURE

the blessing for your father is you . He must be very proud to have childern who love and tend to him that is not always the case and so may of our older family member are left on there on. God bless you and your sister and I pray that your fathers health improves

Glad to help.  Good for you for standing up for your dad, Kathleen!  We have to be our loved ones advocates when they enter the health care system.  If the public had any idea of how many people were harmed or even killed by preventable medical errors every year, they would be shocked. 

 

The Joint Commission (the agency that accredits health care facilities) requires that someone in the facility should be looking at the medication list and making sure it's the right medicine and given the right way throughout every patient's admission, transfers and discharge.  

 

The hospital was supposed to have given you a list of all your dad’s medications and explained them to you when he was discharged.  That list should have also been provided to the rehab facility.  Here's a partial listing of Joint Commission’s National Patient Safety Goals that deal with medication reconciliation:   

 

NPSG.08.01.01: A process exists for comparing the patient’s current medications with those ordered for the patient while under the care of the organization.

 

NPSG.08.02.01: When a patient is referred or transferred from one organization to another, the complete and reconciled list of medications is communicated to the next provider of service and the communication is documented. Alternatively, when a patient leaves the organization’s care directly to his or her home, the complete and reconciled list of medications is provided to the patient’s known primary care provider, or the original referring provider, or a known next provider of service.

 

NPSG.08.03.01: When a patient leaves the organization’s care, a complete and reconciled list of the patient’s medications is provided directly to the patient, and the patient’s family as needed, and the list is explained to the patient and/or family.

 

 

Health care facilities are also required to have a system in place for you to report any safety concerns:

 

NPSG.13.01.01: Identify the ways in which the patient and his or her family can report concerns about safety and encourage them to do so.

 

Here’s a resource that will show you what things to look out for and what kinds of questions you should be asking health care providers.  The campaign is called “Speak Up"  http://www.jointcommission.org/PatientSafety/SpeakUp/

 

If you have future concerns about the way your father is being treated, talk to the bedside nurse first.  If she can't (or won't) do anything, most places will have some kind of Consumer or Patient Relations office.  If it seems like the facility is not taking your concerns seriously, you can call Joint Commission directly at 800-994-6610.  Sometimes the problem is with certain individuals, but at other times, there may be a system issue that administration needs to address.  In either case, something should be done to assure your father is safe and receives good care.  

 

  If there's a staff person who's going out of their way to take good care of your dad, please be sure to write a letter to administration so they can be recognized for their efforts.  There are some wonderful caring people in the medical field and it makes them feel good and keeps them encouraged when they get some positive strokes.

 

This made me remember some years ago, my sister worked in a nursing home and the administrators were so focused on making a profit that they wouldn’t buy enough supplies to take care of the patients.  My tender hearted sister would take money from her own pocket to buy soap and lotion for her patients.  Boy, I could really go on a rant about those folks!
