My insurance (Atena) allowed me to do presurgery visits with my wl surgeon (about 2) and 3 visits with the nutritutionist for weight management and diet counselling. I had to submit proof of my weight for three years which was just a doctor's consult note for one visit per year. I have heard of Drs. asking for as many as 5 years of weight history.
It sounded complicated to me initially, but it turned out to be fairly simple.
Sounds like your problem is that your surgeon would have preferred that you have completed the 6 months of visits with him; so, you've thrown them for a loop.
