Insurance Question - Dr Supervised Plan

My insurance requires 6 mos of doctor supervised weight loss prior to surgery. I have two questions. 1) My doctor (PCP) does not have a weight loss plan. Anyone know of any Phoenix/Glendale, AZ area dr. supervised weight loss plans? 2) What if I do the dr. supervised weight loss plan AND lose weight? Will they not approve surgery because I lost weight through a dr's weight loss plan? Thanks for any help you can give me. Trisha in Glendale

what did you have to provide from weigh****chers? Your original book? A copy of the book? receipts?
