help with letter of appeal!

I am just hoping for guidance or suggestions for a format with me letter of appeal with Cigna select care. My letter of denial states that I 2 appeals to get my surgery approved and I would be crushed if I wasted one with poor information or ignorance on my part. Please help!!!

Hi Trish, You didn't state if you have consulted with a surgeon as yet. If you submitted this paperwork on your own then you need to STOP where you are and find a Surgeon to help you tip toe through the minefield. Mine did all of the reports and went to bat for me. We were approved in less than 3 days on the first request. There is really a lot to this and it is difficult if not impossible to get an approval without medical help.
