Wondering what co-pays there will be just for pre-op

Hi everyone, found out this morning that a lumbar puncture that my nuerologist wants me to have done will cost 125.00 as co-pay. That got me to thinking about what the co-pays will be for my pre-op testing. I know the insurance company could tell me but I have already been on the phone with them twice this morning and it takes forever to get to a human voice. Anybody out their with Pacificare Metro network that can give me any clue?
