NEED INS LTR

I only have one more step before filing for my insurance and could use some help!!! I have to prepare a letter to my insurance company with regard to my weight loss attempts and failures. I am sure that I have seen an example of a similar letter on this computer SOMEWHERE, however, now that I need it I can not find it anywhere. Does anyone know where I may be able to find an example letter? I am hoping to be able to file for my insurance within 2 to 3 weeks and am very excited!!! Any help would be GREAT!!! Thank you!!

Patricia, I didn't see any response to this question you posed. Everyone documents their weight loss differently. One way you might want to do it is to type out a list of dates and weights for each weight loss attempt, and then attach to that photocopies of records of your weigh-ins. Make sense? Patti in Tucson
