new to AZ - 4 yrs post surgery - need doc referrals please

I'm new to AZ and looking for a doctor. I had my surgery 5/9/05 in NY and am due for a post-surgery follow up. I'm looking for a Dr. in the Phoenix area. All recommendations would be greatly appreciated! My insurance is Humana.
