Looking for a Primary Care Physican in Chandler/Tempe

Have switched insurance a few times, and am now with United Healthcare.  I'm at a point in time where I am interested in WLS and would like to have a Primary Care Physican who can guide me through this process.  Any suggestions?

I'm a total rookie at all of this, so any help would be greatly apprecaited.
