Arizona Fill Nurse Practioner

Last year I read about a nurse practioner in AZ who does fills, but I've lost her name and contact information. Anyone know her? Does anyone know of a fill center that takes patients who had surgery in another state? I'm more interested in getting the fills without having to sign up for a big aftercare program. I've had a lot of that already, and I see a nutritionist every two weeks. I just need affordable fills. Any ideas?? Thanks, LAB

Her name is Wendy Hall. She has an office up north somewhere, but I hear she travels to Phx sometimes to give fills. I don't know her contact info. Someone will chime in.
