It's really difficult for anyone (even with the same insurance) to accurately predict whether or not your surgery will be covered. This is because there are so many factors that go into the approval process. A big one is that if you have this insurance through work, your employer may have specifically excluded weight loss surgery from their coverage. The best way to find out if surgery is covered is to call the number on the back of your card, speak with a coverage representative, and ask them specifically which (if any) weight loss surgeries are covered AND what the requirements are.
