At your weight it is likely you have sleep apnea, so you may want to get tested (it could be your second comorbidity).

At 36 BMI, Type 2 diabetes is usually the only condition that pretty much guarantees that surgery will be approved. We always hear that two co-morbidities are needed and I have learned that insurance companies interpret in their own way.
That being said, I agree that you need weight loss surgery and may want to explore other options, such as self-pay if your doctor and insurance do not pay for it. You can go to Mexico and get the surgery for around $7,000. That is a down-payment on a nice car and getting your health back is much more valuable than a car or a fancy vacation.
