Hypothyroidism occurs regardless of obesity. I don't know that it would have the same weight as sleep apnea or diabetes. You need to call your insurance company and/or a bariatric surgeon's office to ask.

Each business employer gets to pick and choose what things they'll cover some will cover based on what you have. Most that do cover it will ask for things like you used for examples. Of course you'll have to prove it and go to the doc and show some history and so on. However some employers do not choose to cover WLS under insurance. Best way to know is to call and or see if any of your co-workers have had theirs covered. I didn't even think about my company covering it, until a work friend told me his was covered the year before.

I agree with Mo, it depends on the insurance policy guidelines. United Healthcare does cover WLS, but you would need to see what is required to qualify. Is it BMI over 40 alone or must you have one co-morbid.
If you have a policy handbook, it will be in there. If not, give them a call and ask them what guidelines do you need to meet to qualify. You are looking for the above info (BMI and a co morbid or just BMI alone), not all the other guidelines, like nutritional consult, psy eval, etc.

I have UHC (private plan with employer). It was covered with BMI over 40 OR BMI over 35 with co-morbidities (like high blood pressure, sleep apnea, etc). They also required a 6 month supervised weight loss period (they told me that this was specific to my company).

It really does not matter what insurance you have, it is based on your employer, you can have UHC and your employer must choose to cover WLS, but someone else who has UHC their WLS may not be covered as the employer chooses to exclude. So it really depends on whether your employer chose to cover this surgery.
