I have an bmi over 40 as well as diagnosis of hypothyroidism, depression and sleep paralysis will that qualify me for surgery?

Every insurance plan is different, and others' experiences will not apply to you.

Find a copy of your benefit book, which you can usually get either online or from your HR department; that will have coverage guidelines.

To get a final answer, call your insurance company, give them your policy number, and specifically ask what is required.

Each business employer gets to pick and choose what things they'll cover some will cover based on what you have.  Most that do cover it will ask for things like you used for examples.  Of course you'll have to prove it and go to the doc and show some history and so on.  However some employers do not choose to cover WLS under insurance.  Best way to know is to call and or see if any of your co-workers have had theirs covered.  I didn't even think about my company covering it, until a work friend told me his was covered the year before.
