UMR Insurance

I have UMR insurance and they are administered by UHC.  I have called UMR and no one can give me a definite answer as to what is needed for approval.  I have contacted my HR department and they have emailed UMR direct to get the exact needs for approval and they have not heard back yet either (it has only been 3 days).  Has anyone used UMR and can you share your experience?

Have you checked online? I was able to view my insurances medical policy online which outlined the coverage and requirements for surgery.

Oh- and if you have the procedure & diagnosis codes it's sometimes easier to get information from your insurance. They plug in the codes and should be able to tell you details to getting that procedure covered. My surgeons office included  the codes in my binder.

I was able to check online but it does not tell me the requirements.  Also the Dr's office called and received information on requirements and when I called with the diagnosis code I received a different answer.  They were told I needed 6 month weight loss history and I was told I did not.  2 coworkers had the surgery last year and the history was not needed but policies change every year.  So that is the problem.  LOL
