I'd suggest calling customer service. Every setup is different, and only the insurance people can give you information specific to your situation.
That said, I don't know of a single person who's been approved without meeting the listed requirements, whatever they may be.

A year or so ago was the first US case of a patient approved for WLS due to diabetes. It took years of fighting to get it approved. To my uncertain knowledge, that's still the only instance of insurance approval without meeting BMI requirements.
6'3" tall, male.
Highest weight was 475. RNY on 08/21/12. Current weight: 198.
