I was rereading my medical policy from my insurance, and the guidelines state that the six-month diet must total six visits over six consecutive months. June would be my seventh month and seventh visit. Technically, I can submit my PCP notes now since I have met the six-month requirement.

I called my PCP and asked him if he could fax his notes to the surgeon's office instead of waiting for a seventh appointment on June 13th. Maybe I can get this surgery done earlier than I had originally thought? 

I just don't see what the problem would be if I have six documented visits. Worst case scenario would be that my insurance company asks for a seventh, but I don't know why they would not accept six documented visits when the policy states that is what they require.

I talked to a customer service rep (again), and she said that December 3 - May 13 would count as my six visits.

My PCP's office called back, and the nurse said he is away for the week. He will sign my letter of medical necessity and send my notes to my surgeon when he gets back. Woohoo!
