I'm confused by my insurance requirements! Help!

ok. So I need 6 months on a supervised diet. I get that. But then my MVP EPO wants 2 years of weight history. Do they mean they want my general weight in 2014 and 2015, or do they want a consecutive 24 months of weigh ins?

Do you have a primary or Dr. that you have seen during the past two years? If so, they weighed you and the bariatric office should take that info as a history of past weight

You should call and ask.

document every conversation with them with date, time and the name of the person you speak with.

My insurance wanted 5 year weight history.  I had my PCP print one record from each year that shows my weight and BMI. It's a good thing they didn't go back farther,  because I hadn't been seeing a doctor yearly before that.

I'm reading 6 months of regular weigh-ins, and make sure you have it clear if they want weekly or monthly weigh-ins or they could make you start over again.

Then they want two years of weight history to show chronic problems with weight.

There is a person in your surgeon's office who is familiar with the insurance plans in your area. Make that person you new BFF. Mine was a GODSEND.  They do this for a living - 5 days a week and can help you with what the insurance company is really looking for and will accept. Although you might need to talk to the insurance company, I would not start with it.

This is prob not the last insurance issue that will crop up, so I would suggest calling the office and seeing what they say first.

My Insurance company (Aetna) required 2 years of weight history and were able to use my initial weigh in at my Consultation from 2015 and a record from Planned Parenthood from 2014. I just had to wait until 2016 to submit it.
