Has my band slipped?

had lap band surgery done 4/21/14, lost 59lbs, have gain about 39 back , stomach very tender to touch, had my band slipped? I feel very bloated a lot , and still get those left shoulder pains , any insight would be appreciated 

Please see a doctor asap. Only a doc will really be able to tell you for sure. 

Yes you must see your surgeon right away, those are all signs of a problem with the band. It could be lots of things but only your surgeon can do the tests you need.

my band was put in 2010 and when I experience pain and alot of discomfort I went to my gastrologist which then I had an endo ddoe and found out it slipped. I then went to my surgeon and I ha it removed and had the sleeve done that was 12/12/14..see your dr. asap...

When it comes to the Band anything out of the ordinary should be checked out just to be safe.  The band can too easily slip or other things go wrong.  Best to catch it early.
