It varies. Some will. Some require payment before surgery, or they won't do it. The only way to find out is to ask.

M1 -24; M2 -21; M3 -19; M4 -21; M5 -13; M6 -21; M7 -10; M8 -16; M9 -10; M10 -8; M11 -6; M12 -5.
It depends on the surgeon. I've had other surgeries where I was billed after the fact and made payments.

You have three entities that are going to be billing your insurance and whom you will have to pay 10% of covered charges to.
- Bariatric Surgeon
- Anesthesiologist
- Hospital.
Your surgeon may require his 10% up front before the surgery is done, but the hospital and anesthesiologist can be paid after the procedure and you can make payment arrangements.
