A little confused

I am New to the process. My insurance is Medicare & I need to have Proof of previous attempts at weight loss. I am not sure how to provide Proof as my old PCP was A Nasty person who didn't help me nor does she recommend Bariatric Surgery. So I would barely go to her. I am however changing to a new PCP next week. I have struggled with weight all my life. My OBgyn of 8 years knows a little about my struggle. I was seeing a nutritionist that she referred me to but now I'm seeing a different lady because my original nutritionist got a different job. Nutritionist feels if I eat more & exercise that's the answer to all of my problems. She ignored everything I told her about chronic pain,fatigue etc. As well as all of my previous attempts only to gain everything back +more. So with that being said I have No clue how to give this information the insurance company is looking for. If anyone has had the same issue please tell me what you did. Also if anyone has Medicare please tell me what you went through. Thank you all in advance

Anything that can be documented counts. Going to weigh****chers, things like that. They should be able to give you more specifics.

6'3" tall, male.

Highest weight was 475. RNY on 08/21/12. Current weight: 198.

M1 -24; M2 -21; M3 -19; M4 -21; M5 -13; M6 -21; M7 -10; M8 -16; M9 -10; M10 -8; M11 -6; M12 -5.

If you have seen the same on/gym for 8 years, they should have a record of your weight at each visit.

I didn't have any written documentation of my many, many diets over the years, but I gave a list of them, including numerous times at WW, Jenny Craig, Atkins, low calories, low fat, blah, blah, blah.  My insurance company accepted my list with no questions.  

Just tell them what you've done over the years, and tell them to get your medical records to see your weight over the years.

5' 5" tall. VSG on August 4, 2015/ Starting weight 239.9/ Surgery weight 210.9/ Current weight 137.4/ Goal weight 140/ No longer overweight, now a NORMAL weight. Now that I'm at goal, it's time to move on to maintenance!!!!!!!!

You don't have to have a conversation with them, or ask their opinion. You just need to get your records which they have to give you... ask the bariatric office the best way to do it.

Medicare doesn't pre-approve, so---who's asking you for proof of previous diet attempts? Your surgeon's office? All I did was list the number of times I'd done Weigh****chers, etc., and gave them copies of my medical records.
